How We Support You

Comprehensive care for complex conditions.

Care Coordination

Your dedicated point of contact for every aspect of care.

Learn more

Your Care Manager acts as the single point of contact who ties everything together—family, caregivers and your medical team—so information travels in one direction instead of getting lost between calls.

  • A thorough assessment of your condition, needs and goals

  • Seamless communication between family, caregivers and your medical team

Medication Assistance

Staying on track with complex medication regimens.

Learn more

Complex regimens are the norm with chronic conditions, not the exception—caregivers track what’s due and when, and can layer in comfort care support alongside it as needs grow.

  • Medication reminders and schedule tracking

  • Paired with comfort care when needed

Disease Education

Understanding your condition is the first step to managing it well—our nurses provide education on its causes, symptoms and treatment options.

Learn more

This isn’t a one-time conversation. Nurses revisit your condition and treatment options as things change, so you’re working from current information, not what you were told at the start.

Self-Management

Knowledge and tools to take control of your health.

Learn more

The goal isn’t dependency—it’s giving you what you need to make your own calls day to day, with caregivers as backup rather than the default.

  • Support making informed, confident decisions

  • Guidance toward a healthier, sustainable lifestyle

Lifestyle Modifications

Small, sustainable changes that add up—personalized strategies for healthy eating, physical activity and stress management.

Learn more

Lifestyle plays a real role in how a chronic condition progresses. Strategies are built around what’s actually sustainable for you, not a generic wellness checklist.

Wound Care

Skilled care for wounds common with diabetes and cardiovascular conditions.

Learn more

Chronic conditions like diabetes and cardiovascular disease raise the risk of wounds that don’t heal on a normal timeline—Registered Nurses and Nurse’s Aides monitor healing closely and adjust care as it progresses.

  • Dressing changes and infection control from Registered Nurses (RNs) and Nurse’s Aides (RNAs)

  • Ongoing wound healing monitoring

Chronic Pain Management

Comprehensive, non-pharmacological strategies—physical therapy, heat or cold therapy and relaxation techniques—to protect your quality of life.

Learn more

Pain management here isn’t just medication—it’s a combination of approaches built around what actually helps you, revisited as your condition or your response to treatment changes.

Emotional Support

A listening ear, and a source of encouragement.

Learn more

A chronic illness affects emotional wellbeing as much as physical health. Caregivers treat that side of things as seriously as the clinical tasks, not as an afterthought.

  • Compassionate companionship alongside physical care

  • A focus on mental health and a positive outlook

Telehealth Services

Care from the comfort of home, when you choose Equinoxe LifeCare for home healthcare in Montreal.

Learn more

Telehealth doesn’t replace in-person care—it fills the gaps between visits, so routine questions or check-ins don’t have to wait for a scheduled appointment.

  • Routine check-ins and medication management by video

  • Virtual consultations with your healthcare providers

Is this a replacement for my doctor or specialist?

No. Care Coordination works alongside your existing doctors and specialists, not in place of them—caregivers keep your medical team informed and help you follow through on their recommendations.

Can caregivers help manage multiple conditions at once?

Yes. Caregivers are briefed on your full file, not just one condition, so care stays coordinated even when more than one issue needs attention at once.

Is telehealth available in addition to in-person visits?

Yes. Telehealth is used for routine check-ins, medication management and virtual consultations between in-person visits—not a substitute for hands-on care when that’s what’s needed.

Can the level of support change as my condition changes?

Yes. Support is never locked in—hours and services can be adjusted as your condition, needs or treatment plan change, without a long-term contract.
