Going Over the Discharge Checklist

Even after a short stay at the hospital, returning home can be challenging.

We ensure everything goes smoothly to support a swift recovery.

Before Going Home

  • checkmark Ensure hospital care is delivered.

Hospitals strive to provide the best care, but sometimes additional support is needed. We can advocate on your behalf and offer extra assistance during your hospital stay.

  • checkmark Send the medical summary to the family doctor.

We can document diagnoses and prescribed medications (names, doses, schedules). We assist in copying and sending medical records and discharge papers to prevent future medical errors.

  • checkmark Schedule follow-up appointments.

We will coordinate the logistics of your hospital care plan and schedule follow-up appointments (dates, contacts, medical requirements) to avoid unnecessary delays.

  • checkmark Obtain necessary medication from the pharmacy.

We can pick up your medication from the pharmacy. We will also inquire about potential side effects and when to seek medical attention, as drug interactions can be unexpected.

  • checkmark Identify symptoms to watch for.

We will provide a list of symptoms to monitor and indicate when to seek medical attention to ensure your recovery is progressing as planned.

  • checkmark Evaluate home safety.

We will evaluate home safety by removing hazards (e.g., carpets) and arranging necessary medical equipment (e.g., walker, wheelchair).

  • checkmark Assist with hospital discharge.

You may need help with hospital discharge (e.g., wheelchair, medical transportation, navigating stairs at home). We are here to assist you.

Discuss your unique needs

Our care managers are available to answer your questions and discuss your options. We will help you find the best care solution for your family.

Maintaining personal hygiene can be challenging for those with reduced mobility. Our caregivers are trained to assist with daily activities such as bathing, dressing, grooming, toileting, and mobility support.

  • checkmark Schedule other regular medical care.

To manage medications, dress wounds, monitor health conditions, and provide other medical care, your dedicated Equinoxe care manager will arrange services with our registered nurses.

  • checkmark Getting out of bed.

To ensure safety while getting out of bed, our caregivers are professionally trained and certified in mobility support and safe full-body transfers.

  • checkmark Going to the toilet.

Our caregivers are certified in mobility support, assisting with trips to the toilet or diaper changes.

  • checkmark Training to prevent fall risk.

Changes in mobility and balance can increase the risk of falls. Our caregivers can train you to move safely around the house and reduce fall risks.

  • checkmark Prepare an emergency plan in case of an unattended fall.

Unattended falls can be serious. We provide 24/7 monitoring to detect falls and will contact your emergency contacts for immediate assistance.

  • checkmark Medication management.

Sticking to a medication schedule can be challenging for seniors. Our caregivers assist with medication reminders, ensuring medications are taken as prescribed.

  • checkmark Prevent heavy object lifting.

Post-operative guidelines often prohibit lifting objects heavier than 10 lbs or 5 kgs for 4 to 6 weeks. Our caregivers can handle heavy objects to support a quicker recovery.

  • checkmark Adapt nutrition to support faster convalescence.

Post-operative guidelines emphasize the importance of staying hydrated and consuming high-protein meals for better healing. Our caregivers assist with meal planning, grocery shopping, and meal preparation, taking dietary restrictions and preferences into account to support a faster recovery.

The Equinoxe team was a lifesaver to my family. They took care of everything we needed to care for my mother and I could not be more grateful. The Care Coordinators and PABs were so sweet and patient throughout the process, and I recommend their services to everyone I know.

Our doctor recommended that we call Equinoxe for help when my father’s Alzheimer became too difficult to manage. Everyone of their staff, from the caregivers to the nurses and care managers, was professional, compassionate and respectful. They kept us informed and consulted with us to make the right decisions for his care. I’m convinced that my father was able to live at home for as long as he did thanks to Equinoxe’s approach. I’ve recommended them, without hesitation, to a few of my friends.

Equinoxe has provided extraordinary services for my family, taking care of my mother after surgery when she was unable to walk on her own. The office has been extremely responsive in helping schedule caretakers to meet our needs, and the caretakers themselves have been kind, patient and supportive of my mom's needs. I highly recommend their services if you are in need of home care.
