Our Equinoxe LifeCare Managers will guide you through our proprietary Equinoxe LifeCare Management Process—the assessment, coordination and supervision of a full range of in-home healthcare services. They are here for you, and act as your personal consultants and advocates—in complete confidentiality and privacy—as you navigate the healthcare system.
Following a comprehensive assessment, our LifeCare Managers will design and manage a personalized care plan that addresses your or your loved one’s medical, physical, social and emotional needs, and unique recommendations to maximize the function and comfort of patients, improve quality of life while monitoring changing needs.
Once a plan is implemented, our LifeCare Managers continue to be your point of contact in an ongoing and evolving capacity. At every step of the way, you are in good hands.
